One page iPad form (simplest)

I’d originally done various apps for a Fire & Safety Consultancy. When he sold that business he became a tattoo removalist and wanted a one page form for clients to fill in on an iPad. There was nothing complex about this. No sharing. No server. No reports. It was the most basic of crm systems, a list of clients and an associated form for the client to fill in bits, read the terms & conditions and sign at the bottom. A new form being created for each session.

It didn’t take much more effort to make it really user friendly.

  • If anyone picked it up and pressed the wake up button it would only ever show a cover page or the form they were filling in.
  • They couldn’t see the buttons at the top.
  • If the owner picked it up, he could enter his name and see all the forms and the buttons at the top.
  • If it was the 1st session for a new client he’d click ‘new client/treatment’ and a blank form would appear that he handed over.
  • If it was a subsequent session he would click ‘find’, enter their name, find all their sessions, and click ‘new session’.
  • This copied all the details from the previous form, except their signature, date and office use only details.
  • Clients were asked to check the details were still valid and sign.
  • When it was signed, automatically a pdf copy was emailed to the client.
  • The client also asked for a button. So that a spreadsheet could be exported.
